Non-12-Step Addiction Treatment
Evidence-based, secular alternatives to 12-step programs for people who prefer a non-spiritual approach to recovery at Keystone Health Group.
While 12-step programs like Alcoholics Anonymous and Narcotics Anonymous have helped millions of people achieve recovery, they are not the right fit for everyone. Some people are uncomfortable with the spiritual framework of 12-step programs, prefer a more science-based approach, or have had negative experiences with 12-step communities. At Keystone Health Group, we respect each client's preferences and offer evidence-based, non-12-step treatment approaches that are equally effective.
Evidence-Based Non-12-Step Approaches
Non-12-step addiction treatment draws on a range of evidence-based approaches: cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), motivational enhancement therapy (MET), medication-assisted treatment (MAT), and acceptance and commitment therapy (ACT). These approaches are grounded in neuroscience and psychology rather than spiritual principles.
SMART Recovery
SMART Recovery (Self-Management and Recovery Training) is the most widely used non-12-step mutual aid program. It is based on CBT and rational emotive behavior therapy (REBT), and focuses on four key areas: building and maintaining motivation, coping with urges, managing thoughts and behaviors, and living a balanced life. SMART Recovery is free, secular, and available in-person and online.
Secular Organizations for Sobriety (SOS)
SOS is a non-profit network of secular recovery groups that emphasize personal responsibility and scientific understanding of addiction. SOS does not require belief in a higher power and is an alternative for people who prefer a secular approach to peer support.
Acceptance and Commitment Therapy (ACT)
ACT is an evidence-based therapy that uses acceptance and mindfulness strategies alongside commitment and behavior change strategies to increase psychological flexibility. In addiction treatment, ACT helps clients accept difficult thoughts and feelings without using substances to escape them, and commit to actions aligned with their values.

Frequently Asked Questions
Is non-12-step treatment as effective as 12-step treatment?
Yes. Research shows that non-12-step approaches including CBT, DBT, and MAT are as effective as 12-step facilitation for most people with substance use disorder. The most important factor is finding an approach that fits your values and preferences.
Can I try non-12-step treatment even if I've tried 12-step before?
Absolutely. Many people try multiple approaches before finding what works for them. Our clinical team helps you identify the approach that best fits your needs and preferences.
