Why Aftercare Is Essential
Research consistently shows that the period immediately following inpatient treatment is the highest-risk time for relapse. The transition from the structured, supportive environment of inpatient treatment to the demands and triggers of everyday life requires careful planning and ongoing support.
At Keystone Health Group, aftercare planning begins on the first day of treatment — not the last. Our clinical team works with each client to develop a comprehensive, individualized aftercare plan that addresses housing, ongoing therapy, peer support, medication management, and community resources.

Our alumni community connects graduates of our program with peers who understand the journey of recovery. Alumni events, peer mentorship, and online community spaces provide ongoing connection and accountability that research shows is essential for long-term recovery.
Alumni Events
Regular alumni events — including sober social activities, educational workshops, and community service opportunities — provide meaningful connection and purpose in recovery. These events celebrate milestones and reinforce the recovery community that is essential for long-term sobriety.
Peer Mentorship
Our peer mentorship program connects people newly in recovery with alumni who have built meaningful lives in long-term sobriety. Peer mentors provide lived-experience guidance, accountability, and hope — demonstrating that recovery is possible and sustainable.
Alumni Check-Ins
Our clinical team conducts regular check-ins with alumni during the critical first year of recovery — providing an opportunity to address emerging challenges, adjust the aftercare plan, and reconnect with treatment resources if needed.
